Description

Hennepin County Health Care for the Homeless (HCH) program is seeking an Integrated Care Clinical Social Worker to join our clinic teams at People Serving People and YouthLink shelters. In this role the Clinical Social Worker will provide short term services in a shelter based clinic setting and in the community. Services will include assessment brief therapy clinical case management service planning resource navigation and referrals for families and youth living with acute mental health symptoms substance use and/or complex physical health needs. Clinical social workers work closely with clinic staff to provide wrap around services and mental health services to patients of the clinic. We are looking for an individual who is skilled and experienced in working with youth and families experiencing homelessness in sheltered and unsheltered settings to provide clinical services and teambased care. This work supports the Health and Human Services mission of improving the health of our communities and the goal of assuring that all peoples basic needs are met.

Location and hours:
This position is hybrid and will be performed in the community onsite at People Serving People 614 South 3rd Street Minneapolis MN 55415 and YouthLink 41 North 12th Street Minneapolis MN 55403 and remote as job duties require. Work hours will typically be Monday through Friday 8 a.m. 4:30 p.m. with some flexibility. While this position is designated as hybrid based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefitearning employees is inperson in downtown Minneapolis. Details about NEO will be provided upon hire.

In this position you will:
  • Work collaboratively with the HCH clinic teams to provide integrated care to patients using the objectives such as building relationships and trust promoting harm reduction while identifying health needs and assessing readiness to engage in healthcare.
  • Provide clinical services such as brief therapy diagnostic assessment and crisis assessment using a harm reduction framework.
  • Provide resource navigation service planning and referral to community partners for ongoing needs.
  • Collaborate with other homeless community provider agencies and Hennepin County departments.
  • Participate in quality assurance and quality improvement projects with HCH clinic teams.
  • Document data plans clinical actions client progress response to care and other relevant patient information in Epic and in compliance with statue and rules of confidentiality.
  • Participate with staff on broader Hennepin County projects.

    • Working with families youth and individuals experiencing homelessness.
    • With mental health and addiction medicine support or working in a health care setting.
    • With intensive case management and care coordination.
    • Establishing community connection and collaboration across agencies and programs.
    • With public health initiatives programs and services.
  • Ability to communicate fluently in Spanish in addition to English both verbally and in writing.
  • Knowledge of:
    • Public health approaches and practices.
    • The impacts of racism on health and the social determinants of health.
  • Ability to:
    • Work in an interdisciplinary team.
    • Communicate effectively in both verbal and written form.
    • Demonstrate understanding of cultural differences and the impacts of racism on service provision organizational effectiveness and on relationships with staff partners and those we serve.

Bilingual candidates who pass a language assessment may qualify for additional differential bilingual pay and duties.
