Job Responsibility Overview: Assessment & Screening: Reviews client history conducts thorough assessments or screenings upon receiving PT orders and communicates findings to clients families and staff; completes required documentation. Planning: Establishes treatment plans with prioritized goals based on assessment; communicates plans to clients families and collaborates with the healthcare team for discharge planning. Implementation: Prioritizes and delivers interventions per plan of care; educates clients/families coordinates with assistants and healthcare team and documents daily treatment activities. Evaluation: Continuously assesses the effectiveness of interventions updates plans of care communicates significant changes and completes discharge documentation. Program Development: Participates in developing new rehab programs supervises student practicums leads training sessions and assists in maintaining rehab policies under leadership supervision.
