Description
We are seeking an experiencedCommunity Outreach specialist tojoin our team at Heartshare St Vincent services. As a member of our outreach team you will play a critical role in identifying and engaging patients in need of followup care promoting access to healthcare and supporting the overall mission of our outreach specialist is expected to enroll a minimum of 30 new members per month regardless of the referral source.
Key Responsibilities
- Checking eligibility criteria appropriateness criteria approval by the lead health home and clearance in MAPP.
- Create outreach letters and mail to all perspective clients for outreach introductions within the 30day outreach period.
- Conduct telephonic outreach attempts and document in the EHR within the 30day outreach period.
- Conduct scheduled and unscheduled home visit appointments to complete enrollment process.
- Outreach specialistis responsible to document all outreach effort and enrollment within the 30day period.
Qualifications
- Associates degreeRequired
- Valid DriverLicenseRequired
- At least six months of outreach experience is Preferred
- The ideal candidate will have excellent organizational and communication skills as well as the ability to read write and speak English fluently.
