SUMMARY OF DUTIES

This Human Services Case Manager position will match the needs of Aging and People with Disabilities in the community with current federal state and local programs or other resources that are available to meet those needs. This is done through case management and by screening applicants for long term communitybased care and nursing homes. This position will also assess eligibility redetermine needs and adjusts benefits and services accordingly. Also this individual will act as an advocate for clients and aging and persons with disabilities within the community.

MINIMUM QUALIFICATIONS

A Bachelors degree in a Behavioral Science Social Science or a closely related field;

OR

A Bachelors degree in any field and one year of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing);

OR

An Associates degree in a Behavioral Science Social Science or a closely related field AND two years of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing);

OR

Three years of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing).

WORKING CONDITIONS

General office environment. You will visit clients hospitals nursing homes APD offices and other providers. A current drivers license is required along with an acceptable driving record.
