As a Community Dietitian with VCH you will:

• Work in partnership with the client and family to establish goals that are safe realistic and reasonable by assessing planning coordinating organizing implementing evaluating and transitioning Clinical dietetic and other services for the client in an interprofessional team model in collaboration with the primary care provider and other care providers.
• Assess service eligibility facilitates the development implementation and adaptation of a care plan delivers direct care and provide care management to assist clients and caregivers to manage their own care and navigate through the various services available within the community setting.
• Work in consultation with other system partners and incorporates current evidence into practice.

Health Services for Community Living (HSCL)

Health Services for Community Living (HSCL) is a communitybased team supporting adults with developmental disabilities who are eligible for Community Living BC services across Vancouver. As part of an interdisciplinary team the Health Services for Community Living (HSCL) dietitian provides comprehensive care planning in community settings such as group homes day programs and family care homes; planning delivering and evaluating care tailored to each clients unique needs; facilitate referrals to other professionals or agencies; provide education; to empower clients families and caregivers with the knowledge and guidance needed to promote wellbeing and foster independence; collaborate with a wide range of partners including CLBC community service providers and health care teams across Vancouver Coastal Health.

Education & Experience

• Current full registration as a Dietitian with the College of Health and Care Professionals of BC (CHCPBC).
• Two years recent related experience or an equivalent combination of training education and experience.
• Restricted Activities A & C are required to practice.
• Valid BC Drivers License. Local area travel requires use of personal vehicle.

Knowledge Skills & Abilities

• Broad knowledge of Clinical Dietetic theory and practice within a client and family centred model of care.
• Broad knowledge of the CHCPBC standards of practice and guidelines for clinical practitioners.
• Basic knowledge of Clinical dietetic standardised assessments interventions and measures.
• Knowledge of enteral nutrition.
• Knowledge of management of adults with eating drinking and swallowing disorders (dysphagia) as well as the skills to teach recommended diet and fluid textures to clients/care givers.
• Knowledge and demonstrated skill in the provision of evidencebased clinical practice related to designated client populations.
• Knowledge of provincial acts and regulations related to home and community care.
• Knowledge of acute chronic disease palliative and health management and selfmanagement support.
• Knowledge of community resources available for client/family support and related health services.
• Knowledge of transitions through continuums of care.
• Knowledge of other health care professions and their role in client care.
• Knowledge of research process and methodology.
• Knowledge of adult education principles methods and tools.
• Demonstrated ability to promote clientfocused care that demonstrates care for and with clients and significant others sensitive to diverse cultures and preference client advocacy and social justice concerns.
• Demonstrated ability to work collaboratively as a member of a professional team.
• Demonstrated ability to adjust to new or unexpected events.
• Demonstrated ability to manage psychosocial and behavioural issues.
• Demonstrated ability to communicate effectively through verbal nonverbal written electronic and social media.
• Demonstrated ability to employ effective conflict resolution and reconciliation approaches and techniques.
• Demonstrated ability to teach facilitate and coach.
• Demonstrated ability to employ teaching/learning strategies to transfer knowledge to engage clients/families in planning and self management of care.
• Demonstrated ability to promote staff morale cooperation assertiveness creative planning for change and innovations implementation of policies or other protocols and ongoing professional development of self.
• Demonstrated ability to develop rapport trust and ethical relationships with clients/families family care providers and other health care professionals.
• Demonstrated ability to plan organize schedule and prioritize work.
• Demonstrated skill in the use of equipment and in the techniques appropriate to Clinical Dietetic treatment.
• Ability to operate related equipment and current administrative and clinical computer applications.
• Demonstrated physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy all health care workers working in publiclyfunded health care facilities are required to report their past receipt of certain vaccines or history of certain infections.
