Privacy Notice for California Applicants: for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker in the development and implementation of the discharge plan. Maintains and coordinates Medicaid state submission forms for presentation to onsite reviewer and files them accordingly or as per facilitys procedure. Assists with case management referrals discharge planning as assigned by interviewing patients/families followup on required assignments and organizing the development of a safe patientfocused discharge plan. Provides clear verbal and written communication of all discharge planning information to stakeholders

Qualifications

EDUCATION EXPERIENCE TRAINING


Required qualifications.


1. Knowledge in discharge planning/utilization management/case management terminology and functions in both managed care and nonmanaged care environments.
2. Active state LVN license required upon hire and maintain current or other State License in a related field.
3. Current BCLS (AHA) or (ARC) certificate required upon hire and maintain current.
4. Experience in basic to intermediate computer skills/knowledge.
