Overview:



Job Description Summary:


Works with individuals groups or communities to facilitate positive and healthy behaviors. Interacts with adolescents who have congenital heart disease using a theorybased lifestyle intervention to increase physical activity.


Job Description:


Essential Functions:

  • Conducts assessments to identify behaviors and develop intervention plans.
  • Monitors and evaluates the effectiveness of interventions and modifies plans as needed.
  • Collaborates with other professionals such as psychologists and social workers to provide comprehensive care.
  • Provides training and support to caregivers and other professionals to ensure consistency in intervention strategies.
  • Maintains accurate and uptodate records of client progress and communicates with relevant parties as needed.
  • Maintains accurate and timely documentation of all interventions and progress.


Education Requirement:

  • Bachelors degree in a relevant area including exercise physiology nursing physical therapy psychology community wellness social work or another healthrelated field required.
  • Be pursuing an advanced degree in a relevant area as mentioned above.
