JOB TITLE: APRN OR Physician Assistant

EMPLOYMENT TYPE: Fulltime; 40 hours/week or Parttime; 20 hours/week

SCHEDULED HOURS: MondayFriday flexible

PROGRAM/LOCATION: Pathways Milestone and New Life Center Putnam CT

ABOUT THE PROGRAMS:

This position is responsible for providing services to our 3 programs located in Putnam CT:

Pathways: Opiate Treatment Program that provides MAT (primarily methadone) individual therapy group counseling psychoeducational groups individualized treatment planning recovery coaching and ongoing assessment and referrals

Milestone: a 30day intensive level 3.7 residential treatment center designed to accommodate and treat women with a history of substance abuse trauma and mental health issues.

New Life: A Residential Substance Abuse Treatment program designed to accommodate women with substance abuse and cooccurring mental health disorders who have infants toddlers under the age of 4yo and/or expectant mothers

DUTIES & RESPONSIBILITIES:

  • Take a complete medical history and do a physical examination on patients as clinically appropriate.
  • Conduct a psychiatric evaluation and initiate or continue medications for mental health disorders
  • Assess and manage postacute withdrawal symptoms
  • Initiate and continue evidencebased medications for treatment of substance use disorders including but not limited to methadone and buprenorphine
  • Prescribe medications as indicated in agency policies and procedures
  • Perform annual physical exams on patients being treated in the outpatient methadone clinic as needed
  • Independently evaluate diagnose and treat or refer for care of acute illnesses including but not limited to: upper respiratory illnesses urinary tract infections sexuallytransmitted infections acute back pain skin wounds uncontrolled hypertension asthma exacerbations uncontrolled diabetes etc.
  • In consultation with the physician(s) and care team members develop plans for further evaluation of health needs e.g. referral to other health team members or specialists.
  • Provide counseling and support as clinically appropriate in areas of social problems recognizing the emotional component of illness making referrals to mental health professionals as needed.
  • Educate patients about disease prevention health maintenance and wellness
  • Provide oversight of the RNs who are carrying out physician orders
  • Additional duties include preparing medical records clinical reports and correspondence.
  • Guide and coordinate aspects of care as an engaged and active member of a multidisciplinary treatment team

QUALIFICATIONS:

Education: APRN (Family Nurse Practitioner AdultGerontology Primary Care Nurse Practitioner AdultGerontology Acute Care Nurse Practitioner Psychiatric/Mental Health Nurse Practitioner) or PA (Physician Assistant) training program in accredited college or university

Experience: At least 3 years of postgraduate experience as a Nurse Practitioner (APRN) or PA (Physician Assistant) working with duallydiagnosed clients and ability to manage mental health and substance use disorders. MAT experience preferred.

Licensure/Certification/Registration: CT APRN license or PA license. Active DEA required.

Training: Must conform to prescribed standards for continuing education as set down by appropriate state or national advisory boards.

COMPENSATION*:

$121356/minimum and up commensurate upon experience

*Actual rates are determined at the time of offer and are based on relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or signon incentives. (Please note for parttime positions this range would be prorated based on hours worked)
