Department: Shared Living
Full Time
Location: Springfield
Schedule: MondayFriday 9a5p
Base Salary: $48000$50000/year $2000 differential per additional case (See details below)
Key Responsibilities:
- Manage and oversee the support of individuals in Shared Living homes
- Coordinate and monitor services provided by Care Providers Individual Support Staff (ISS) Personal Support Staff (PSS) and Respite Providers
- Ensure all services align with funding regulations agency policies and quality standards
- Identify stressors and implement interventions to maintain placement stability
- Supervise support staff and provide crisis intervention as needed
- Collaborate with the Department of Developmental Services (DDS) and other agencies
- Complete all required reports and assessments in a timely and professional manner
- Broad knowledge of human services or social work
- Bachelors degree in a related field preferred
- 35 years of progressively responsible experience
- Strong organizational and communication skills
- Computer proficiency (Microsoft Office and related systems)
- Valid drivers license and reliable transportation
- Starting Base: $48000 $50000/year based on experience education and workload
- Differential: $2000 for each additional case
- Minimum Caseload: 15
- Maximum Caseload: 20
