Position Summary:
Chemik Hospice Care LLC is seeking a compassionate and skilled Hospice Social Worker to provide emotional support counseling and community resource coordination for patients and their families facing terminal illness. As a vital member of the interdisciplinary team (IDT) the Hospice Social Worker helps ensure patients receive holistic personcentered care that honors dignity preferences and quality of life.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments within 5 days of admission and as needed.
- Participate actively in the interdisciplinary group (IDG) to help develop and revise individualized plans of care.
- Provide emotional and psychological support to patients and families throughout the endoflife journey.
- Assist families with advance care planning funeral planning and navigating difficult decisions.
- Coordinate access to community resources such as financial assistance housing food transportation and caregiver support.
- Provide crisis intervention and grief support before and after death including participation in the bereavement program.
- Educate patients and families about hospice philosophy services and their rights.
- Maintain timely and accurate documentation in the electronic health record (EHR).
- Collaborate with chaplains nurses physicians and other team members to ensure quality coordinated care.
- Participate in quality improvement initiatives and inservice training as required.
Qualifications:
- Bachelors Degree in Social Work (BSW) required; Masters Degree in Social Work (MSW) preferred.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of Insert State.
- Minimum of one 1 year of experience in a healthcare or hospice setting preferred.
- Strong communication problemsolving and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid drivers license and reliable transportation for home visits.
Working Conditions:
- Hybrid role including patient homes skilled nursing facilities and office settings.
- May involve occasional evening or weekend visits as needed.
- Requires ability to handle emotional situations and provide support in times of grief and loss.
