Join Our Team at ValleyWide Health Systems Inc.

We are seeking a compassionate and detailoriented Community Support Specialist to join our team. This position is responsible for managing and monitoring daytoday patient caseloads and providing Community Support Services to help individuals coordinate care overcome barriers and improve their overall wellbeing.

Key Responsibilities:

  • Provide care coordination and life skills training for patients with significant mental illness.
  • Manage caseloads for both Community Support Services and Meds Only Program patients.
  • Develop implement and monitor individualized treatment plans.
  • Conduct assessments document patient interactions and maintain accurate clinical records.
  • Coordinate care in clinic settings patient homes and community environments.
  • Schedule and attend appointments for Restoration Program patients if assigned.
  • Collaborate with clinical teams to support patient care goals.
  • Attend team meetings individual supervision and group trainings as scheduled.
  • Ensure timely documentation (within 48 hours) and meet productivity and compliance standards.
  • Demonstrate cultural competence and sensitivity in all interactions and documentation.
  • Participate in secure patient transports when needed.
  • Uphold HIPAA requirements and maintain strict confidentiality at all times.

Qualifications & Skills

Required:

  • High School Diploma or equivalent.
  • Lived Experience Personal history of recovery from addiction mental health condition trauma or caregiving for someone with such conditions.
  • Certification as a CPFS NCPRSS or equivalent (or ability to obtain within 1 year of hire).
  • Willingness to obtain a Bachelors degree within a designated timeframe after hire.

OR

  • Bachelors degree (preferred) in Social Work Counseling Psychology or related field.
  • Candidates with nonrelated degrees may be considered with one year of relevant experience.

Additional Qualifications:

  • Valid Colorado drivers license and ability to travel as needed.
  • Ability to pass a Behavioral Health background investigation.
  • Strong written and verbal communication skills.
  • Proficient in Microsoft Office and general office equipment.
  • Demonstrates empathy professionalism and cultural sensitivity.
  • Excellent time management and organizational skills.
  • BLS Certification (provided upon hire).
  • Teamoriented with a collaborative mindset.
