Job Description

DaVita has an open position for a Social Worker who will be a vital member of each patients core care team. Life on dialysis can be a difficult transition for many patientsand youll be there to support and advocate for them. In this vital role you will help patients understand their rights and responsibilities and guide them in managing the physical mental emotional and financial demands of End Stage Renal Disease.

Requirements for the Position:
Must have minimum LSW.
Essential Duties and Responsibilities:
Conducts reviews and assesses quality of life survey results develops patient goal setting completes appropriate follow up to analyze patients plan to improve scores and quality of life.
Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing.
Counsels new and current patients and families on treatment options and modalities.
Conceptualizes problems interventions and the evaluation of interventions with respect to human behavior personality development and social environment.
May be required to cover more than one facility in the area.

Compensation for the role will depend on a number of factors including a candidates qualifications skills competencies and experience and may fall outside of the range shown.
