Position Summary
This Bilingual Case Management Coordinator position is with Aetnas LongTerm Services and Supports (LTSS) team and is a fieldbased position out of Hillsborough County and surrounding areas in FL. The requirements is for candidates to live in Tampa Florida and to travel 5075 of the time to meet with members face to face.
We are seeking selfmotivated energetic detail oriented highly organized techsavvy Case Management Coordinator to join our Case Management team.

Case Management Coordinator is responsible for telephonically and/or face to face assessing planning implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the members overall wellness. Case Management Coordinator will
effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and longterm outcomes as well as opportunities to enhance a members overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to members health needs; including but not limited to: Prior Authorizations Coordination with PCP and skilled providers Condition management information Medication review Community resources and supports.
Required Qualifications
Bilingual Fluent English and Spanish (reading writing and speaking)
Must reside in Tampa FL
Must possess reliable transportation and be willing and able to travel up to 5075 of the time o meet members face to face in Hillsborough county and surrounding areas in FL. Mileage is reimbursed per our company expense reimbursement policy
1 year of experience in behavioral health or a longterm care setting
Preferred Qualifications
Ability to multitask prioritize and effectively adapt to a fastpaced changing environment
Managed care experience
Case management and discharge planning experience
Education
Bachelors Degree in Social Work or related field

The typical pay range for this role is:
$21.10 $36.78This pay range represents the base hourly rate or base annual fulltime salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience education geography and other relevant factors. This position is eligible for a CVS Health bonus commission or shortterm incentive program in addition to the base pay range listed above.
