Certified Community Behavioral Health Clinic
- Provide lifechanging care coordination for clients with persistent mental health and substance use challenges both in the community and in the office.
- Coordinate care with mental health and health care providers to create a seamless integrated care experience.
- Assist clients in navigating services such as legal aid housing employment substance use treatment medical care and more helping them overcome barriers to successful recovery.
- Collaborate with your team to ensure each client receives the support they need to thrive.
- Build partnerships within the community making it easier for clients to access the resources they need.
- Participate in trainings as needed.
- A BA/BS in Social Work or a related field.
- At least 3 years of experience in care coordination.
- Someone who is compassionate an excellent communicator and ready to make a difference.
- The ability to travel with a valid drivers license and reliable vehicle.
