Patient Care Manager (Hybrid) Housing Support Focus
Location: California
Employment Type: Remote
About the Role
We are seeking a fulltime Patient Care Manager based in California to work remotely while providing inperson housing support for patients as needed. This role focuses on assisting patients with housing stability social services navigation and ongoing care management. The goal is to enhance patient wellbeing improve access to essential resources and address housingrelated healthcare disparities. We specialize in medically complex patients and are looking for compassionate professionals who understand housing as a critical social determinant of health.
Compensation & Benefits
Salary: $75000 $80000 annually
Paid Time Off: 3 weeks 2 weeks 67 federal holidays)

Responsibilities
Assist patients in securing and maintaining stable housing including support with housing applications financial assistance programs and landlord negotiations.
Connect patients with local state and federal housing resources such as HUD programs Section 8 and emergency rental assistance.
Work collaboratively with healthcare teams case managers and social workers to integrate housing support into care plans.
Educate patients on financial and economic resources to improve overall health and stability.
Provide regular updates to the care team regarding patient needs barriers and progress.
Qualifications
Bachelors Degree (preferred) Associates Degree (required)
2 years of experience in housing support services (required)
Strong knowledge of federal and state programs including disability benefits food assistance and IHSS
Bilingual candidates encouraged to apply
