Hours Status: FullTime Hourly 8a5p MF
Starting Pay Range: $15.87 hourly
Position Summary:
Works with clients to assess plan implement monitor and evaluate actions required to meet the clients health and human services a member of the Promoting the Integration of Primary and Behavioral HealthCare (PIPBHC) Team.
- Identify clients needs and barriers to receiving the appropriate care
- Facilitates access to appropriate providers through inquiry referral coordination and timely follow up
- Drafts service plans with clients to establish goals and objectives; reviews case progress and determines case closure
- Works collaboratively with Integrated Care staff and Primary Care Staff to achieve positive client outcomes
Requirements:
- Bachelors degree in human services related field.
- Must have a valid drivers license reliable transportation and present proof of automobile insurance.
Skills:
- Proven work experience in Case Management or related job
- Previous experience with psychological aspects of care
- Problem solving time management and organizational
