Case Manager Summary of Duties:Our Case Managers are dedicated to overseeing patient discharge planning and coordinating the overall care plan to ensure a smooth transition from the healthcare setting to the next phase of care. They are responsible for assessing planning implementing and coordinating services that meet the patients health needs across the continuum of care with a focus on discharge planning and social work interventions.
The Case Manager collaborates with multidisciplinary teams including physicians nurses social workers and the patients family to facilitate timely and effective discharge processes. They ensure that all necessary referrals are made and that appropriate resources are in place for the patients postdischarge care. The Case Manager acts as a liaison between the patient family and healthcare team ensuring clear communication and a comprehensive care plan that meets the patients unique needs.
Successful Case Managers excel in relationshipbuilding and demonstrate strong judgment and problemsolving skills particularly when addressing complex discharge planning. They are creative and flexible ensuring that care is delivered in the most effective timely and costefficient manner while adhering to regulatory requirements.
The ideal candidate must be detailoriented goalfocused and possess exceptional criticalthinking analytic and timemanagement abilities. They should have a compassionate and empathetic personality along with the ability to work collaboratively within a multidisciplinary team. Strong communication and interpersonal skills are essential as is the ability to consistently maintain a professional and supportive approach throughout the discharge process.
