Job Description

Landmark Hospital of SW Florida located in Lehigh Acres has an exciting opportunity for a RN Case Manager to join our dedicated and professional team.

Shift:Days/FT

Summary:Responsible for the overall coordination of the patients plan of care and the resources utilized for the patients stay. Assesses plans implements coordinates monitors and evaluates options and services to meet an individuals health needs throughout the continuum of care.

Provides discharge planning through referrals and multidisciplinary planning and serves as a liaison between patient/family and the healthcare team to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks related to the coordination of care. A certain degree of creativity and latitude is required. Ensures that the patients unique needs are met in the most appropriate timely and costeffective manner possible.

The ideal candidate must be highly detail and goaloriented with superior analytic criticalthinking and timemanagement skills. They must have a positive and empathetic personality with the ability to work effectively with others as part of a multidisciplinary team in complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude.

Requirements:

  • Registered Nurse license
  • 2 years of Case Management experience
  • Case Management Certification including CCM ACM or NACCM preferred
  • Ability to develop and maintain positive working relationships
  • LTAC/Acute Care Hospital experience strongly preferred
  • Strong computer skills preferred
