This position is open to current employees of Pueblo Regional Center ONLY.



Description of Job

About the Position:
The purpose of this position is to give support and services to people in the areas of health safety and activities of daily living while also providing opportunities for people to achieve their desired personal outcomes as described in their Individual Plans. This position will act as a Mentor to all designated staff that provide direct care services to our individuals. Duties in this position include:

Provide & Assist with Daily Living Care and Support:

Mentoring:
  • provide mentoring to employees who are assigned to this program; up to 50 of this duty statement will be spent providing guidance and direction regarding direct care services while performing the tasks during essential functions of the job. The position will demonstrate related competencies and observe return demonstrations provide onthespot training and evaluate and document that each competency was demonstrated proficiently
  • provide followup communication with assigned employee(s). Communication should be weekly (facetoface via phone or email) to answer questions and provide ongoing support for the first month. Monthly communication should occur after one month has passed throughout the next six months
  • position will be assigned to work in ratio (or out of ratio when needed for mentoring) at varying locations to provide support to staff (or teams) that are struggling
  • provide training and demonstration in methods and techniques that are approved and supported for individuals (or teams) that struggle to be successful
  • make observations and recommendations according to their training and expertise and will report back to management and the Mentor team (Staff Development) regarding the need for further support
  • may be pulled to multiple work areas within a shift and will drive to each location.
  • trains newly assigned staff in work duties and schedules
  • provides onsite training and monitoring of the residents activities of daily living to ensure timely and accurate implementation of program plans as identified in the Individual Program Plan
  • identifies and reports to the direct supervisor any problems that arise in the area of staff compliance

Community Integration:
  • provide/assist in the implementation of learning support or experience programs
  • Report any issues/concerns
  • monitor area/individuals to ensure a safe environment
  • recommend alternate opportunities to encourage learning growth or stabilization
  • consider the uniqueness of each individual that receives support and services and facilitates opportunities for selfdetermination and active participation in the management of that individuals life
  • transport individuals receiving services to/from appointments scheduled events and emergency transports
  • coordinate with other staff/departments for assistance to transport persons to/from treatment scheduled/nonscheduled events and/or appointments

Home Care & Meetings:
  • refine the physical condition of the living site by addressing safety concerns making purchases decorating the living area and performing housekeeping duties
  • report any safety/hazard issues
  • attend assigned meetings and inservices as scheduled by the supervisor to fulfill the mission/vision of the agency
Other Duties as Assigned:
  • other Duties as assigned
  • may be required to work other locations and/or shifts as needed to address agency resource needs
  • must be annually certified in physical intervention techniques

Conditions of Employment:
  • A preemployment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions conviction of crimes of moral turpitude or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges. Additionally you will be required take a drug screen.
  • Shift Work PRC is a 24/7 facility. Person in this position must be available to work any shift any day and/or any area to meet Agency needs.
  • Valid Drivers License license must be valid and unrestricted and remain so throughout employment. Inability to drive a state vehicle could result in a loss of employment.
  • Travel Position must travel to remote work sites and must utilize a State vehicle if available. If not available must utilize personal vehicle and submit for mileage reimbursement
  • Preemployment Physical/Tests
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.

An appeal or review must be submitted on the official appeal form signed by you or your representative.
