The Residential Case Manager is responsible for the overall coordination enrollment and monitoring of needed CHRP waiver and nonwaiver services including but not limited to social emotional medical dental clinical and transporting or arranging client transportation. Will collaboratively work with counselors clinicians and supervisors to stabilize the childs condition improve behaviors and help develop skills that enable them to transition back to a less restrictive communitybased environment whenever possible. Ensures that client needs are met including clothing hygiene products school supplies and other items as necessary and will include managing pass/visitation schedules participate in client staffings the creation and implementation of treatment plans and discharge planning. Case manager will be QMAP trained and responsible for administering documenting medication and adhering to medication management protocols when necessary.


Essential Job Functions

  • Collaborate on admissions and discharge process for residential clients including but not limited to completing necessary paperwork coordination of services scheduling exit interviews and other duties as required to include but not limited to discharge planning.
  • Participate in client staffing meetings gather and assess information about clients needs and desired treatment outcomes for individualized client driven goals.
  • Meet with clients regularly to assess and ensure treatment engagement and progress.
  • Assist clinicians with discharge wraparound plans and secure outpatient appointments.
  • Coordinate and effectively communicate with clinicians and Residential Director regarding client case status needs and providing individualized client driven goals.
  • Coordinate with medical staff on medication orders creating MARs and communicating medication changes to staff.
  • Support clients during crisis and act as a client advocate when needed to support the client gains toward treatment goals.
  • Maintain client files including authorization for evaluation and treatment transportation medical release insurance verification and care custody and control and other required documentation.
  • Provide onthefloor training support coaching to client and staff to help residential clients gain measurable skills increase prosocial behavior improve functioning and demonstrate ability to maintain a stable living arrangement in the community.
  • Administer medication to clients or ensure medication is administered appropriately by qualified staff member when necessary.
  • Must be able to work some evenings and/or weekends.


Other Job Functions

  • Coordinate treatment for children between cottage milieu education therapy medical department recreation and volunteer coordinator.
  • Ensure that medical and dental appts are met within time frame. Medical w/I 14 days dental with in 8 weeks of admission if needed.
  • Review for new client orientation checklist completed within 24 hours of admission to RCCF.
  • Supports utilization of a behavior management system that is strengthbased developmentally appropriate and that focuses on teaching childrens new skills.
  • Provide timely debriefing for any critical incidents as needed.
  • Complete incident reports and debrief forms as needed.
  • Respects cultural differences of all clients and staff. Promotes inclusivity at all times.
  • Contributes to a positive work culture and strengthsbased treatment program.
  • Other related duties as assigned.


Physical Requirements

  • TCC requires staff in direct care of children to be able to perform physical restraints or containments as necessary. Clients are age range 518.
  • Assume a restraint for periods of time varying from 530 minutes.


Working Conditions

  • This position is on campus.


Minimum Qualifications

  • B. A. in Human Services Psychology or other related field with experience working with children in the child welfare or a minimum of five years prior experience working with children with developmental disabilities.
  • Supervisory experience a plus.
  • Must have or ability to obtain current QMAP certification & knowledge of agency medication protocols.
  • Strong organizational and time management skills.
  • All candidates must have (or qualify to obtain) a current Colorado drivers license.
  • All positions require criminal CBI background check FBI fingerprint check State of CO child abuse check DMV driving record check preemployment drug test preemployment physical academic credential verification and reference check.

Benefits

*New Hire Sign on Bonus: $500 after 90 days; additional $1000 after 6 months
