The Opportunity Alliance is looking to fill a fulltime Case Manager role with our Wraparound program.
Our HighFidelity Wraparound program works with youth aged 1220 with a diagnosed mental health disorder and/or developmental delay who are involved with the Juvenile Justice system. Wraparound provides comprehensive holistic youth and familydriven intensive case management services through the development of a team comprised of both professionals and natural supports. The Wraparound team seeks to support the family/youth in developing individualized goals so that they can maintain in their homes and communities and achieve their vision of a fulfilled life.
The Case Manager is a direct service staff position for the community services area. This position contributes to the accomplishment of the Agencys mission through the provision of homebased and communitybased case management services to youth and families with a mental health diagnosis or developmental disability.
This Case Manager attends monthly staff meetings and participates in weekly supervision with the designated supervisor.
Work is performed reviewed and evaluated under the Program Director.
Schedule: Fulltime 40 hours per week. Monday Friday
Location: Office is located in South Portland but services will be provided throughout Cumberland County and north.
