Title: Targeted Case Manager Adults
Worksite: Barren Service Center
County: Barren
Hours Status: FullTime MF 8a5p
Pay Range: $15.87 hourly
Brief Job Description:
Advocate and provide assistance to adult clients experiencing severe mental illness and/or substance use disorders.
Responsibility includes accessing community resources build professional relationships with community agencies provide crisis assistance maintain contact and monthly supervision etc.
Qualifications:
- Bachelors degree in human services (Psychology Social Work Sociology Family studies Counseling Public Health Special Education Education.
- Oneyear postgraduation minimum experience working with SMI adults; or one year of case management experience; or 6 months if possesses masters degree in approved field.
- Valid drivers license and current automobile insurance.
