Opportunity
The Health Pathway Navigator (HPN) will provide culturally responsive 11 inperson or remote wellbeing navigation to clients experiencing persistent health challenges as well as language and cultural barriers that prevent them from achieving settlement outcomes through health care system & resource navigation & needsbased referrals. The HPN will also focus on supporting refugee and immigrant families experiencing health issues such as autism physical or mental disability mental health disorders PTSD HIV infectious diseases deafness and chronic illnesses. Additionally the HPN will develop accessible health servicesrelated resources for dissemination among newcomers while actively updating resources with new information and feedback from clients. The HPN will support the delivery of groupbased inperson Healthy Settlement Journey information & orientation (I&O) workshops for newcomers from equity groups on various healthrelated topics such as IFHP MSP how to find a family doctor immunization & other general health topics.
