Hours:MondayFriday 8am 430pm Dayshift. NO NIGHTS HOLIDAYS or WEEKENDS!
Overview
As anIntegrated Care Managerwith Sentara you get to work together in an intensely collaborative way with the patients family and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This positions primary duty is customizing what is best for the patients. That would include helping with things like home health longterm care placement home medical equipment homeless and community resources. You will find that teamwork is exceptional with everyone working together to ensure the best care for our patients. Education
- Diploma RN/ADN/BSN/MSN
Certification/Licensure
- RN License Virginia/Compact/Multistate
- Basic Life Support (BLS) required within 90 days from hire
Experience
- 3 years experience RN

The base pay rate for Full Time employment is:$76731.20$127878.40Additional compensation may be available for this role such as shift differentials standby/oncall overtime premiums extra shift incentives or bonus opportunities.Legal Resources Plan
