Job Requirements:
- Provides direct social work services to patients in their homes
- Works with the interdisciplinary team in integrating social work services into the plan of care
Education
- Masters Degree in Social Work from an accredited School of Social Work
Required Licensure/Certification
- Must have Maryland License in Social Work (LCSWC) with counseling experience
- Must obtain current CPR certification
- Must have valid Maryland Drivers License
Work Experience
- Home Care experience is preferred
- Experiencewith management of adult population
- Clinical experience in counseling and casework for the terminally chronically and acutely ill adultpopulation
