Salary: $31.01 per hour.
Permanent: Parttime 17.5 Hours per Week Evening Hours and Weekends will be required.
Location: The Social Worker will have a combination of work locations. The majority of work will be required in office some days out in the community with clients and the potential to work from home occasionally. The schedule will change based on client and program needs.
The Social Worker for the Gender Affirming Health Clinic will work in all active offices including Aurora located at 15150 Yonge Street. On the days of onsite work the candidate will be required to travel within York Region & South Simcoe for client meetings and staff training or other meetings.
Job Profile:
The Gender Affirming Health Clinic is a service that aims to create a safe confidential and inclusive space. We provide culturally competent and genderaffirming care to trans nonbinary and gender diverse people. We work collaboratively with our community partners to ensure supportive and comprehensive care to clients and their families. Our team uses a holistic and compassionate approach when providing services and supports.
The Family Service Worker works with the families of clients who needing support with gender identity transitionrelated supports and other mental health needs. The Family Service Worker works collaboratively with the program team to identify strengths and needs of family members to develop longterm goals to support the clients recovery. The Family Service Worker will provide psychoeducation and system navigation to build the family members understanding of psychosis related conditions and the impact it has on daily functioning. The importance of the family role in a persons recovery is well documented. The family worker supports caregivers in building healthy relationships enhancing personal connections implementing selfcare strategies becoming strong advocates for their loved ones and in developing a sense of belonging and support in their community; The Family Service Worker provides support and education to parents so that they are best able to support their loved ones recovery. This support is provided in addition to the ongoing support that is provided by the clients case manager.
The Family Service Worker will carry a caseload clients families who will require support and who may require counselling. Attention to the social determinants of health and how this impacts family experience is critical. The Family Worker also values biopsychosocial approaches to mental health treatment plans.
As families play an integral role in recovery but are not the actual clients and do not always have similar needs or views on treatment the need for effective communication between all parties is essential for which the family worker is largely responsible to facilitate. The Family Service Worker works strategically with the case manager to support the clients recovery goals and helps the family understand the client is empowered and encouraged to make their own health decisions.
Responsibilities:
Assess and identify the familys needs in conjunction with the family client and case manager
Assist families in creating support plans using a biopsychosocial framework
Provide individual counselling support and education to family members
Plan and deliver client and family groups and support client and family attendance at groups
Keep current on best practices as it relates to family support
Prepare familyoriented resources for distribution
Maintain a caseload recognizing the need for extensive travel in some rural areas and intensiveness of contact with specific clients
Work closely with the multidisciplinary team to offer perspectives and develop and implement the treatment plan and transition plan. Including identifying and linking to community resources and supports as needed
Participate in program orientation as required
Conduct initial interviews that include collaboration with the client in taking a psychosocial history outlining demographics (gender age language) health (psychiatric medical recent/current treatments) informally assesses mental status assesses current strengths and needs and helps the client to identify goals and informal and formal supports and services.
Inform the client and family of confidentiality requirements and the limitations of confidentiality.
Identify any cultural or other issues and responds in a sensitive manner.
Participate in case conferences in collaboration with the client and family.
Provide brief focused family consultation. Provide counseling support concerning illness and symptom management problem solving conflict resolution interpersonal skill building and goal attainment.
Provide information and education to family members other agency personnel and community members concerning Recovery principles and practices mental health illness coping strategies and available resources.
Promote and teach health and wellness principles and practices and assists the family to access and use primary health care services within and outside of the program.
Takes direction from clinical supervisor manager and director. Able to amend services and interventions in consultation with the Manager as well as provide information and feedback to the Manager concerning client/family needs.
Takes part in group supervision and professional development activities.
Requirements:
Minimum 3 years experience providing supportive counselling to individuals and groups.
Minimum 3 years experience working with community health or social service organizations with a emphasis on CommunityBased Mental Health.
This position reports to and is supervised by the Manager People and Teams.
Working Conditions: Office work community settings meetings in clients homes and travel with clients.
