Job Description
Find your purpose with CentraCare by providing care and treating patients where theyre most comfortable In their home. As a Home Health Case Manager Registered Nurse you will manage a case load of approximately 2025 patients and care for 56 patients per allows you to give each patient the personalized attention and quality care they deserve in a less stressful duties include wound management infusion therapy assessments education and training for patients on their new diagnosis and more. We provide extensive training and ongoing RN office support while youre making home visits to patients. This position will be working in Annandale Maple Lake Clearwater South Haven Kimball and surrounding areas.
Schedule:
- Fulltime 72 hours every two weeks(10 scheduled days)
- Days Monday Friday 8:00 am 4:30 pm
- Every 4thweekend Two holidays per year
Pay and Benefits:
- Pay based on experience and degree
- Associate/Diploma in nursing starting at $37.27 per hour
- Bachelors in nursing starting at $38.20 per hour
- Masters in nursing starting at $39.16 per hour
- Pay range: $37.27 $55.91 per hour
- Milage reimbursement and extra pay for working weekend shifts
- $3500 hiring bonus for eligible candidates

Qualifications:
- Graduate from an accredited school of nursing required
- Current Minnesota Registered Nurse license required
- Current American Heart Association (AHA) BLS Healthcare Provider card required
- Minimum of one year of RN experience or 2 years experience in a Home Health setting
- Reliable transportation required
- Valid Minnesota drivers license a driving record in good standing and insurance liability coverage meeting CentraCares standards required
Based out of our office in Marketplace Mall in Waite Park the Home Health department primarily takes place in the communities we serve in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo Alexandria Little Falls and more. Teams are assigned to areas to minimize driving.
