A fantastic new opportunity has arisen in our Swindon service for an Outreach Workersupporting rough sleeping individualsand those at risk of homelessness into and throughout their treatment journey.

You will be the key liaison between CGL services local hostels and shelters the local council and partners and will actively engage individuals with the services they need to support improved health wellbeing social inclusion and appropriate accommodation.

Location:Swindon

Full Time Hours: 37.5 per week

Full Time Salary Range: 27861.26 32002.35 (pro rata for part time hours)

Responsibilities

Purpose of role:

  • To engage with a range of individuals who have adopted streetbased living including rough sleepers and beggars with a view to engaging them in appropriate services and moving them into more settled lifestyles.
  • Increase service user choice service user involvement and empower service users to maintain sustainable change; working collaboratively and proactively to ensure that services are fully coordinated and are working collectively towards the achievement of goals and positives outcomes for each individual client.
  • To build and develop service users personal strengths and social networks in the UK and/or in the individuals country of origin.
  • To provide a range of flexible and effective interventions to engage with hard to reach service users.

About you:

  • Prior knowledge and experience of working with people who are homeless sleeping rough or sofa surfing
  • Be an excellent communicator both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions services and good practice
  • Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others

Fulltime hours at Change Grow Live are 37.5 hours per week. For parttime roles the salary and payments will be pro rata based on contracted hours.
