Service Coordinator
My Health My Resources of Tarrant County (MHMR) is looking for a skilled Service Coordinator. Each person receiving services in the HCS TxHmL and GR CFCin Tarrant County will have a single identified staff called a Service Coordinator (SC). The SC will assist the person in identifying and articulating his/her priority outcomes personal goals and hopes for the future as well as his/her requests and need for services and supports. The SC will elicit the persons feedback coordinate monitor the services for desires outcomes as identified in the Person Directed Plan (PDP). Thegoal of a Service Coordinatoris to provide the people we serve with seamless consumer driven services and supports. The SC must also be knowledgeable about the array of services and supports available in the community.
Minimum qualifications:Bachelors Degree in Health or Human Services
Acceptable Degrees Include: Applied Behavior Analysis Behavioral Services field Communication Disorders Criminal Justice Counseling Education Family Studies Gerontology Educational Psychology Health Services Human Services field Human Growth and Development Medicine Nursing Physician Assistant Psychology Rehabilitation Social Services field Social Work Sociology and Special Education
Required License
Valid Texas drivers license with an acceptable driving record
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
Preferences
Bilingual a plus but not required
