Care Coordinator Domiciliary Care

40 hours per week oncall 28000 (rising postprobation)

Our client a wellrespected and growing provider of domiciliary care is excited to be opening a new domiciliary care branchand theyre looking for an experienced and compassionate Care Coordinator to play a key role in its success.

This is an exciting opportunity for someone who thrives in a fastpaced peoplefocused environment and wants to help shape a service from the ground up. The successful candidate will be central to delivering safe highquality care that enables people to live well in their own homes.

As a Care Coordinator you will work closely with the Registered Manager to ensure care packages are planned and delivered effectively schedules are wellorganised and clients receive the consistent personcentred support they deserve

.

Due to the nature of the role candidates must hold a full driving licence as regular travel within the community will be required.

Required Skills & Qualifications

  • Level 3 (or above) in Health and Social Care
  • Previous experience in the care sector ideally in a Care Coordinator or team leader role
  • A confident communicator with strong organisational skills
  • A proactive problemsolver who can manage changing priorities
  • Full UK driving licence and access to a vehicle

Key Responsibilities

  • Meet regularly with service users to review and ensure care aligns with their individual care plans
  • Train and assess care staff in the safe and effective administration and management of medication
  • Coordinating care visits and rotas to meet service users needs
  • Managing client referrals and care planning alongside the Registered Manager
  • Supporting and supervising care staff including conducting spot checks and field visits
  • Handling oncall duties on a rota basis (shared within the team)
  • Ensuring the quality and compliance of care provision in line with CQC requirements
  • Building strong relationships with service users families and professionals
