The Bronx Point Supportive Housing Program has opportunities for Case Managers to join a multidisciplinary team. Bronx Point provides permanent housing for 82 formerly homeless individuals and families. Tenants have experienced homelessness and many have serious mental health and/or and substance misuse challenges. Case Managers are integral to our team and are responsible for helping chronically homeless individuals transition into permanent housing with supportive services. Case Managers provide intensive case management services and referrals for medical care psychiatric care community living skills financial management and substance use treatment. Case Managers assist our clients with a wide range of activities to develop daily living skills and referrals as well as help the clients stabilize mentally medically and socially. In addition Case Managers assist tenants with identifying accessing and maintaining benefits including housing subsidies SSI/SSDI and public assistance. To accomplish all of these goals Case Managers must build rapport with tenants who often have experienced intense trauma.
Bronx Point opened in December 2023 and is staffed by a small dedicated multidisciplinary team. Bronx Point provides a high level of support to its staff. Weekly supervision is provided as are clinical rounds and trainings. A Spanish Bilingual speaking case manager is preferred but not required as about 20 of tenants are monolingual Spanish speaking.
The ideal candidate demonstrates the ability to work in partnership with clients from a strengthsbased perspective overcome stigma and navigate complex systems. Additionally the ideal candidate adapts to an active environment manages crises practices exceptional time management has strong writing and communications skills exhibits excellent attention to detail and remains a team player.
