The Colorectal Surgical RN Navigator is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decisionmaking. Assumes responsibilities and accountability for planning guiding and evaluating the delivery of patient care. Monitors patients progress throughout treatment. Intervenes as needed to ensure compliance and optimum care. Collaborates within a multidisciplinary team to facilitate timely appropriate screening diagnosis therapeutic intervention treatment and supportive care across the healthcare continuum. The RN Navigator assumes responsibility for the coordination of all resources necessary to provide care. Utilizes advanced expertise to identify implement and improve processes to provide an integrative model of care to the patients while assuring the highest level of patient satisfaction. The RN Navigator provides patient education for each patient regarding their disease process and treatment throughout the healthcare continuum. Demonstrates and promotes professional development. Maintains a professional safe and therapeutic environment for rendering care. The RN Navigator will work with the colorectal team to establish policies and procedures and coordinate in the Cancer Center. Will also assist in the application process to achieve National Accreditation as a Rectal Cancer program.
NYS licensed/registered or eligible RN required. Bachelors degree in Nursing or healthcare related field three 3 years post licensure care RN experience (preferably in specialty) OR an AAS in Nursing and five 5 years post licensure RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills required. CPRBLS certification required by the time of appointment.
Three 3 years as a Certified Nurse/Patient Navigator in a specialty area preferred.
