Job Description

BASIC FUNCTION:

Under the supervision of the SBIRT program manager is responsible for screening assessments Brief Intervention Brief Treatment and Referral to Treatment. This is a 85pm position and will require providing services at off site locations. Must be relaible dependable and able to work independently.

MINIMUM QUALIFICATIONS:

  1. High school diploma or equivalent. Associates preferred.
  2. Bilingual Strongly Preferred
  3. Must have a lCDC license in good standing
  4. Well versed in various treatment modalities such as Motivational Interviewing or CBT.
  5. Well versed about multicultural communities and working with Latino populations.
  6. Excellent verbal and written communication skills
  1. Clear driving record to escort clients to activities if needed.
  2. Keeps required training current: Nonviolent crisis intervention Abuse Neglect Exploitation and others as applicable.

PRINCIPAL ACCOUNTABILITIES:

  1. Conduct all necessary procedures including screening & assessment.
  2. Develops positve working relations with colloborators
  3. Is responsible for the overall care of assigned clients.
  4. Completion of assessment of psychosocial histories and the development of a master treatment plan for client if indicated
  5. Performs assignments
  6. Attends weekly treatment team meetings unless otherwise instructed.
  7. Provides and facilitates individual sessions.
  8. As trained documents via progress notes or administrative notes client response to treatment and that clients are working on identified problems.
  9. Attends and actively participates in training sessions.
  10. Is responsible when indicated to contact client families for scheduling of appointments.
  11. Knowledge of highrisk behaviors in adolescents (and adults) such as: selfharm; eating disorders; video technology and gambling addictions; alcohol vaping and other related substances
  12. Continues accruing necessary classes and experience hours required for certification and licensure.
  13. Is responsible for scheduling all appointments for clients including lab work physical/medical and nutritional assessment and/or any other referrals needed.
  14. Is responsible for providing his/her assigned clients with an individualized aftercare plan upon discharge
  15. Maintains client confidentiality per AAMA policies and maintains professional relationships with clients at all times.
  16. Completes all required data entry and paperwork accurately per timeframes established.
  17. Completes GPRA with minimal errors
  18. Performs other duties as assigned.

Ability to professionally and effectively communicate and interact with the population served. Ability to handle stressful situations in a tactful safe way. Ability to document accurately. Ability to work with sensitive information and maintain confidentiality. Ability to build rapport with clients but maintain professional boundaries to assist them with achieving their program goals. Ability to communicate effectively orally and in writing. Ability to remain calm and make informed wellthoughtout decisions at all times. Ability to effectively deal with and help resolve client crisis situations. Ability to collaborate well with a team of caregivers to ensure the best quality care of clients. Ability to read write speak clearly and use a computer telephone and other office equipment. Ability to effectively communicate with adolescents parents counselors faculty administration teams System of Care team members and collaborating organizations including community support providers. Ability to provide intervention including individual group and crisis counseling.

Office setting primarily occasional travel to other locations.

WORK SCHEDULE: This position is nonexempt and is eligible for overtime. Typically hours are 8:00am to 5:00pm however are subject to change based on client and program needs. On occasion may be asked to participate in special events that may occur on the weekend.
