Job Purpose:
As a Nurse (RN/LPN) the Navigation Specialist is responsible for coordinating internal and external patient needs including transports scheduling and internal supplemental rounds. This role will serve our patients and families with love and respect. The Navigation Specialist will remain flexible and responsive when changes occur in patient activity and workload to support team while maintaining a strong sense of patient and family focus. To be safe honest and committed to bettering the lives of those we serve and provide a positive and loving space for those we encounter. Responsible for coordinating the health care needs of the patient. Connects patients and families to resources and support systems assists with interaction between healthcare providers performs supplemental internal rounds schedules appointments streamlines paperwork and acts as a patient advocate.

Qualifications:
Education:
Completion of diploma Nursing Program (LPN or RN)
Experience:
1Year experience as RN or LPNis required
License/Certification/Credential:
RN/LPN: Current LPN or RN license in state of certification within 1 year of employment. Valid drivers license. Current in check off for med pass and sedation.
