Job Description

You will be contacted directly by the 3rd party staffing agency representing this facility after applying to this is a 52 week contract working with the Bob Stump VA Medical Center in Northern Arizona to help provide care to Veterans in critical need. There is potential to extend each year for up to 5 years in total. Typical shifts will be 8 hours each day with a general Monday Friday 8 5 schedule. Site of ServicePotential locations include: Chinle VA ClinicKayenta VA ClinicPage VA ClinicPolacca/Hopi VA ClinicTuba City VA Clinic Position SummaryAs a Mental Health Nurse Practitioner you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic or medical center as directed by physician or midlevel provider. You will collaborate with the core PACT Team (Primary Care Provider RN and Medical Assistant) and expanded PACT Team including family/caregiver VA and communitybased services involved in developing the patient care plan. Core Responsibilities Evaluates veterans for mental health disorders and appropriately diagnoses and treats mental health conditions. Orders and performs diagnostic tests and therapeutic procedures as related to mental health diagnoses and treatment and interprets test results for deviations from normal and provides follow up care. Prescribes medication and recommends dietary and activity programs. Performs appropriate interval follow up visits for necessary treatment of mental health diagnosis. Refers patient to the VA medical specialist when indicated. Counsel patients on the use of prescription medications and educates patients in relation to treatment plan. Develops and implements patient management plans assists in provision of continuity of care through the PCMM (Primary Care Management Module). Evaluates patients records from medical providers outside the VA and works with the veterans utilizing rules set forth by the VA for comanaged care. Conducts individual counseling/therapy. Contributes to group therapy (i.e. PTSD groups; weight management; smoking cessation; grieving; and personal interactions). Contributes to pain management plan as needed. Involves veterans in planning and assuming responsibility for activities. Refers veterans when indicated to community resources and other organizations. Addresses issues such as advance directives and organ donation in accordance with acceptable standards. Inputs all patients information into VISTA/CPRS. Assists in comanaged care collaboration with nonVA providers and facilities. Maintains confidentiality of veterans data and information. Complies with all VA and company training requirements. Fulfill compliance requirements of the Office of Inspector General (OIG) Joint Commission(JC) Environment of Care (EOC) oversight lab compliance and other related items. Remain focused on achieving excellent clinical outcomes through the specified VA guidelines. Must provide excellent customer service to each veteran and his/her family both in person and over the phone as well as to fellow colleagues and clinic visitors. Participate in the clinics outreach events to help support the clinics enrollment initiatives. Support patient enrollment and retention in appointments followup visits or nurse visits. Embrace and support new initiatives whether clinical or operational Requirements / QualificationsGraduate of an accredited school of nursing and graduate of an accredited program for nurse practitioner (MSN required) including preceptorshipPsychiatric Mental Health NP Certification by applicable professional organizationMinimum oneyear clinical experience as an adult mental health CRNP. (Specific requirements could vary based on individual VA contract).Must be credentialed and remain in good standing through the Veterans Health Administration (VA).Must comply with and maintain all requirements for a valid unrestricted license in the state of desired employment or in any U.S. state or territory depending on VA contractual certification in Basic Life Support (must be renewed periodically as specified by the certifying agency AHA valid for two years e.g. and in ACLS as specified by individual VAMC contractCurrent unrestricted Drug Enforcement Administration (DEA) registration This job is CONTINGENT upon contract award client approval completion of a favorable background investigation and the ability to obtain and maintain a Public Trust / High Risk clearance.
