Job Overview


The PersonCentered Plan (PCP) Specialist is responsible for supporting the coordination of services for up to 7585 people. The PCP Specialist facilitates the development of service plans that reflect individuals unique strengths needs preferences and aspirations. The PCP Specialist works to ensure the timely completion of all PCPs on their assignment list. The PCP Specialist works closely with regional teams to review usage of services as they create and revise plans as needed to match trends and needs of people served. This position is also expected to support the overall operation of Abilities Network when needed. Certain tasks may be asked that do not fall into the essential functions of this job.


The hourly rate of pay for this fulltime position is $21.00 and is nonnegotiable.


Essential Functions


1. Collaborates with individuals their family team members and other professionals to create personalized PCPs. These plans outline goals strategies and timelines for achieving desired outcomes.

2. Maintains and ensures that all routine PCPrelated documentation is completed on time.

3. Tracks the PCP process in the agencys information system to ensure implementation and documentation adheres to state regulations and agency quality standards.

4. Troubleshoots errors or delays in the PCP approval process and works to mitigate errors or develops an action plan to ensure timely approval of PCPs so no individuals receiving agency services experience delays in services and autoextensions of plans are minimized.

5. Monitors LTSS daily for pending plan activity and acts accordingly.

6. Monitors LTSS billing exceptions and tracks items to their resolution to ensure that services can be billed against each PCP.

7. Compares requested service units as outlined in each persons plan as submitted to data entered into LTSS and confirms accuracy.

8. Engages with Coordinators of Community Services (CCS) other service providers and DDA on a routine basis to keep PCPs moving forward following up on any errors and working with teams and leadership to resolve issues.

9. Ensures goals and implementation strategies fit within the services the agency provides.

10. Stays current regarding services offered through the Community Pathways Waiver and the Community Supports Waiver.

11. Collaborates with Regional Services Managers and Director of Outreach regarding new admissions ensuring the accurate service authorization requests and development of PCP related goals that were discussed during the intake process.

12. Stays current regarding DDA required PCP processes and documents ensuring all documents are the most recent version.

13. Monitors PCP review completion by due dates.

14. Monitors the clarification request process and ensures timely resolution of issues.

15. Demonstrates a strong understanding of Abilities Networks Mission Values and Concepts for Reflection & Growth.

16. Performs other related duties.

Requirements

Required Education and Experience


1. Degree in human services social work or related field required

2. Minimum 3 years of experience working with adults with intellectual and developmental disabilities


Physical Demands

This role is primarily sedentary and is a remote based position. Travel is required to attend meetings etc.
