Summary
Parkview Health is accepting applications for a clinical pharmacy specialist in ambulatory care with a focus in population health.
The population health pharmacist will support a growing pharmacy presence within Population Health and our Clinically Integrated Network (CIN). This pharmacist will work as part of a team that includes two existing pharmacists as well as three pharmacy technicians. The ideal candidate must be passionate about engaging with patients be selfmotivated have excellent time management skills and be highly flexible. Technology skills including a strong working knowledge of Excel and the ability to help build and refine workflows are essential. Dedication to building and maintaining knowledge of Population Health concepts Parkviews CIN and valuebased contracts as well as concepts and technical specifications related to quality measures (such as HEDIS STAR and ACOs) is critical. The candidate must demonstrate excellent verbal and written communication skills to provide excellent service to all customers including patients families pharmacists pharmacy technicians and other health care professionals including systemlevel leaders.

Position Qualifications: Candidates must possess a pharmacy degree from an ACPEaccredited institution and have completed a PGY1 pharmacy residency. A PGY2 residency in Ambulatory Care is highly preferred. Pharmacy licensure or eligibility in Indiana is required. Candidates must have a high level of clinical practice skills enthusiasm for teaching and the interpersonal skills needed to interact effectively with students health professionals and patients. Board certification in Ambulatory care is required within 2 years after hire or as soon as eligible per Board of Pharmacy Specialties requirements.
