Job Description
Summary
Parkview Health is accepting applications for a clinical pharmacy specialist in ambulatory care with a focus in population health.
The population health pharmacist will support a growing pharmacy presence within Population Health and our Clinically Integrated Network (CIN). This pharmacist will work as part of a team that includes two existing pharmacists as well as three pharmacy technicians. The ideal candidate must be passionate about engaging with patients be selfmotivated have excellent time management skills and be highly flexible. Technology skills including a strong working knowledge of Excel and the ability to help build and refine workflows are essential. Dedication to building and maintaining knowledge of Population Health concepts Parkviews CIN and valuebased contracts as well as concepts and technical specifications related to quality measures (such as HEDIS STAR and ACOs) is critical. The candidate must demonstrate excellent verbal and written communication skills to provide excellent service to all customers including patients families pharmacists pharmacy technicians and other health care professionals including systemlevel leaders.
Essential activities will include:
- Maintaining an indepth knowledge and understanding of relevant medicationrelated measures and their impact on our valuebased contracts.
- Navigating and manipulating reports from insurance companies to support workflows and patient outreach.
- Completing patient outreach including telephone calls and electronic communication to:
- Assess potential reasons for medication nonadherence and determining a potential root cause for medication nonadherence.
- Educate patients on various topics such as strategies to improve medication adherence the importance of statin therapy and osteoporosis screening and treatment.
- Evaluate complex cases identified via consult.
- Evaluating care gaps related to statin therapy and engaging with patients and providers to develop a pharmacotherapy plan to close statin gaps whenever possible.
- Coordinating with other key stakeholders to close care gaps for medication adherence statin or osteoporosis measures when applicable.
- Identify develop and execute opportunities for valuebased contracts where pharmacy services could impact cost of care utilization or quality measures.

Parkview offers a progressive pharmaceutical care environment in a variety of practice settings with tremendous growth in ambulatory pharmaceutical care including primary care hospitalbased clinics specialty clinics and a retail/specialty pharmacy. Inpatient services include interdisciplinary rounding and surgical/critical care and oncology satellite pharmacies. Parkview Pharmacy leadership includes a team with dedication to each facility and service type including Inpatient and Ambulatory Clinical Services Distribution Services Managers for Parkview Regional Medical Center and Parkview Hospital Oncology Manager and Retail & Specialty Managers. Additionally there are supervisors in each area to help support daily needs and department projects in conjunction with lead pharmacists and lead technicians.
Position Qualifications: Candidates must possess a pharmacy degree from an ACPEaccredited institution and have completed a PGY1 pharmacy residency. A PGY2 residency in Ambulatory Care is highly preferred. Pharmacy licensure or eligibility in Indiana is required. Candidates must have a high level of clinical practice skills enthusiasm for teaching and the interpersonal skills needed to interact effectively with students health professionals and patients. Board certification in Ambulatory care is required within 2 years after hire or as soon as eligible per Board of Pharmacy Specialties requirements.
