Position Description:

Our amazing APPs work closely with our doctors to provide wellcoordinated teambased care!

Join a group of providers who provide profound and comprehensive care to patients in an outpatient setting we apply scientific knowledge and clinical expertise for the diagnosis treatment and compassionate care of adults. We are well trained in both common and complex illnesses and have a particular focus on the care of patients with multiple chronic medical conditions.

The Medical College of Wisconsins Department of Medicine Division of General Internal Medicine is seeking a Nurse Practitioner or Physician Assistant to provide a broad range of diagnostic and therapeutic services for the patients of the Primary Care Outpatient Services located in Milwaukee Wisconsin at Froedtert Hospital Specialty Clinics.

Our providers help patients understand disease prevention and the treatment of conditions including cardiovascular disease endocrine disorders cancer and musculoskeletal disease. We also support patients with chronic diseases by partnering with other specialists to coordinate care. By working with our nursing staff social workers and community health workers we can deliver comprehensive care at an elevated level.

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration patient care patient safety research quality improvement and teaching.

The Ideal Candidate will be an experienced Primary Care APP who is looking to be a part of a fastpaced clinic and who wants to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW). Areas of responsibility will include but not be limited to providing direct patient care to an adult population in an outpatient setting.

In this role you will:

  • Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring general medicine intervention in an outpatient environment as a member of the Internal Medicine Clinic Health Care Team.
  • The team is responsible for providing management of primary care health and wellness urgent care assessments and the treatment of comorbid conditions such as but not limited to: diabetes hypertension heart failure asthma and anemia in an adult population.
  • Writes accurate and timely progress notes procedure notes and/or admission and transfer orders.
  • Facilitates and communicates with patient family and staff to promote continuity of care across the continuum of care.
  • Utilizes current and evidencebased knowledge to inform treatment planning and clinical decision making in the management of wellness womens health acute and chronic conditions.
  • Conducts posthospital discharge and ED followup evaluations.

Core Advanced Practice Responsibilities

  • Perform comprehensive histories and physical exams identifies normal and abnormal findings including physical cognitive and psychosocial status.
  • Order perform and interpret screening and diagnostic procedures and tests and appropriately communicates and documents information
  • Write accurate and timely progress notes procedure notes and/or admission and transfer orders
  • Develop pharmaceutical and nonpharmaceutical treatment plans.
  • Monitor effectiveness of plan of care
  • Educate patients on disease and treatment plans.
  • Perform procedures as trained and appropriately credentialed.
  • Document within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborate and/or consults with physicians and other health care providers as appropriate.
  • Educates counsels and assists patients and families regarding current health problems health maintenance activities and preventive behaviors.
  • Attend required training and meets key regulatory requirements including those of the privileging institution CMS and the Joint Commission.
  • Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field through educational programs inservices professional associates and current literature.

Position Requirements:

Preferred Experience: 2 years of APP Primary Care experience preferred; 5 years of APP experience a plus!

Minimum Education:

Advanced Practice Nurse: Masters Degree or higher in Nursing
Physician Assistant: Bachelors Degree or higher in Physician Assistant Studies

Minimum Requirements: *or eligible

Advanced Practice Nurse Practitioner Wisconsin Physician Assistant License

Wisconsin RN License Certification by NCCPA

Wisconsin Advanced Practice Nurse Prescriber License AHA ACLS AHA BLS DEA
