Short Description
- Collaborates with providers within the Department of Psychiatry and attends case conference meetings.
- Knowledge of DCP&P reporting and collaboration as well as use of collaterals
- Provides psychosocial assessments (crisis intervention) psychoeducation social work consultation referrals and Case Management.
- Collects patient and family information and history including education mental health substance abuse and medical issues to determine patients current needs.
- Ensures referrals to the appropriate agencies or organizations to connect patients to care.
- Knowledge of local resources to suggest to families including resources for food security rent and utility assistance mental health counseling etc
- Interface with local area schools to communicate patient needs and coordinate home bound instruction
- Ability to request send and expedite medical records
- Documents all notes in EMR
- Provides supervision to BA/BSW staff as needed. LCSWs provide clinical supervision to LSW and MSW staff as assigned by their Director
- Other duties as assigned
Experience Required
12 years health care experience
Education Requirements
Requires a masters degree in social workMSW)
License/Certification Requirements
Current NJ SW License (NEW JERSEY SOCIAL WORKER) preferred LCSW Preferred.
