The Social Worker provides direct patient care under the guidance of a designated healthcare professional in accordance with federal state and local regulations and within policies procedures and guidelines of Bon Secours Mercy Health (BSMH). May provide clinical and administrative supervision.
Essential Job Functions
Provides individual and family psychotherapy
Group facilitation participates in family and treatment team meetings
Discharge planning
Community resource knowledge and collaboration
Psychosocial assessments
Knowledge of involuntary processes; Knowledge of guardianships advanced directives and POAs
Substance use disorder (SUD) screenings
SUD discharge planning
Preadmission Screening and Resident Review (PASRR) completion
Postdischarge calls
Patient health questionnaire (PHQ9 ColumbiaSuicide Rating Scale (CSSRS) and social determinants of health (SDOH) screenings
Crisis assessment and referral services
Successfully participates to complete BSMH and departmental specific training as

Licensing/Certification
Licensed as a Social Worker in state of practice (preferred required within6 months of hire)
Education
Bachelors Social Work (required)
Masters Social Work or Social Administration (preferred)

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patients status and interprets the appropriate information needed to identify each patients requirements relative to his or her age specific needs and to provide the care needed as described in departmental policies and procedures.
