About the Role:
We have an exciting fixedterm opportunity as an Outcomes Lead to drive the operation of a Supported Independent Living (SIL) home in Lilydale Victoria. In this pivotal role within our Home and Living Team you will mentor and guide a team of Disability Support Workers to deliver highquality personcentred outcomes to our residents. You will work closely with the individuals we support their families/guardians and staff to ensure the highest level of care and positive outcomes.
You will be well supported by an experienced and committed Outcomes Manager and our Practice Quality and Safeguards team who are experts in Zero Tolerance human rightsbased positive behaviour support and a personcentred approach. Their guidance and coaching will empower you to lead with confidence.
This SIL home provides 24hour active support to five individuals with varying support needs. You will lead a team of 13 permanent Disability Support Workers operating on a 4week roster. Additionally you will be supported by a Complex Health team to provide tailored support including enteral feeding epilepsy management behavioural support oral suction and other complex health care needs.
Responsibilities:
- Safeguard the human rights safety and wellbeing of individuals ensuring they are free from abuse.
- Build partnerships with service providers including clinicians community organisations funding bodies and regulatory agencies to create opportunities for supported individuals.
- Stay updated on legislation NDIS Service Standards and best practices through professional development.
- Lead by example demonstrating personcentred practices to staff for individuals daily routines and outcomes.
- Supervise and manage staff overseeing plans programs and performance management.
- Support skill development in staff to meet the unique needs of individuals.
- Ensure staff assist individuals in maintaining relationships pursuing social educational and vocational opportunities and participating in their communities.
- Support individuals in using their preferred communication methods and maintaining optimal health.
- Assist with recruitment monitor NDIS budgets and manage staff rostering aligned with service agreements.
- Maintain documentation on individual outcomes and support the implementation of Personal Outcomes Measures (POMs).
- Respond to enquiries from people with disabilities and their families providing appropriate referrals.
- Perform other tasks as directed by the Outcomes Manager.
About you:
- A track record of effectively leading teams of staff to perform their roles to a high standard and to provide innovative and progressive service delivery models.
- An understanding of both physical and intellectual disabilities and other complex presentations (including mental health support).
- A good understanding of Positive Behaviour Support (PBS) and personcentred practice.
- Experience in supporting people with a disability and their families. Ability to advocate on behalf of people with intellectual disabilities.
- Experience in working effectively with people who have complex communication needs
- Strong interpersonal skills and ability to form effective working relationships with colleagues management families and external bodies.

Requirements:
- Certificate IV in Disability or equivalent
- A current NDIS Worker Screening and Working with Children Check (we can support you through the process of obtaining one or both checks if you dont already have them).
- Current First Aid Certificate with CPR
- Evidence of COVID19 vaccination status including booster
