On an Average day you will:
Act as a central hub for communication ensuring team cohesion and patientcentered care
Keep systems organized effective and efficient to ensure smooth operations and exceptional outcomes
Support the clinical team by managing workflows assisting with records and optimizing productivity
Collaborate with supervisors on staffing logistics and patient services
Handle nonclinical inquiries organize departmental mail inventory and equipment
Proactively prevent avoidable hospitalizations and ER visits
Promote Elaras mission values and policies in all you do
Maintain patient and team privacy and confidentiality in line with HIPAA regulations
And most importantly bring people together to deliver exceptional care
What Youll Need:
Education: High School graduation or GED preferred to have college degree
1 year of experience in a hospice or home health setting
Strong understanding of medical terminology and healthcare workflows
Experience supporting or collaborating with a clinical management team
Proficient with office equipment and basic tech tools (computer fax copier etc.
Familiarity with Medicare/Medicaid guidelines (preferred)
