Case Manager Mental Health Professional HOPWA

$1000 Sign on Bonus

The Association for Individual Development (AID) is a nonprofit organization whose mission is to empower people with physical developmental intellectual mental health challenges; those who have suffered a trauma; and those at risk to enjoy lives of dignity and purpose. We are looking for aCase Manager Mental Health Professional HOPWA in Elgin IL who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Schedule:

Option 1: Fiveday workweek MondayFriday
Option 2: Fourday workweek TuesdayFriday

What you will be doing

  • The CM MHP is responsible for managing individuals housing including: all paperwork needed to remain in compliance with TCHIP/HOPWA program requirements/rules assisting individuals in finding and obtaining appropriate housing maintaining leases developing and maintaining good working relationships with landlords providing medical case management and behavioral health services as appropriate.
  • The CM MHP is expected to obtain and maintain all needed trainings to remain in compliance with TCHIP/HOPWA requirements. The CM MHP is expected to participate in ongoing meetings and any other required committees needed to facilitate good working relationships with the HOPWA team as a whole including surrounding area HOPWA teams.
  • The CM MHP must complete documentation of services including mental health services and HOPWA related services provided in accordance with state and agency standards as applicable.
  • The CM MHP is expected to work with a team of behavioral health service providers within the agency as well as outside agencies if applicable to ensure positive clientcentered approach.
  • This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management.

Essential Job Responsibilities:

  • Work scheduled hours and be flexible to meet client and program needs as assigned by Program Manager or Director.
  • Assure compliance with all agency state and federal regulations while providing services and completing assignments. Review and follow updated policies and procedures.
  • Facilitate communications and coordination of services with other AID staff and professionals in the community.
  • Maintain professionalism and good boundaries when working with clients coworkers and outside agencies.
  • Meet minimum service hour provision productivity standards (MRO) monthly.
  • Complete all required case management documentation (assessments IMCANS consents TCHIP/HOPWA paperwork residential paperwork) on a timely basis.
  • Complete and sign all MRO Documentation within 48 hours using Cx360
  • Complete Case Worthy documentation by the 2nd of each month for the previous month.
  • Meet with all assigned clients on regular basis depending on program and client needs; turn in completed daily activity logs.
  • Develop review and revise the IMCANS and complete corrections within the timeframe allotted.
  • Complete assessment tools and necessary paperwork (including Locus SAMA consents client rights and privacy practices) per program procedure.
  • Assist clients with activities of daily living including apartment maintenance and provide client training consistent with the treatment goals.
  • Provide guidance to MHP and RSA staff regarding implementation of IMCANS goals/objectives.
  • Obtain Input from clients families guardians and other staff on how to improve residential services.
  • Monitor and report safety of clients living arrangement
  • Acquire and maintain required trainings and certifications as well as any other trainings assigned by Manager.
  • Assist clients in paying bills budgeting and banking per program procedure.
  • Assist client with completing TCHIP IDPH rent renewal and other housing paperwork in a timely manner to help maintain clients housing
  • Oversight or management of client funds when necessary (dual signature accounts AID rep. Payees).
  • Obtain and maintain client benefits (Social Security Medicaid Link Card Etc).
  • Assure client records are properly maintained per agency procedures including scanning necessary paperwork into Cx360 per procedure.
  • Complete authorizations reauthorizations and spenddown paperwork in a timely fashion.
  • Update Cx360 with corrections whenever necessary but at least annually to ensure accuracy of records.
  • Provide effective services for clients individual needs and in line with client rights.
  • Attend monthly clinical supervisions per DHS requirement
  • Attend monthly team meetings and be a positive contributor.
  • Recognize emergency situations and take appropriate action. Contact Manager and Director per procedure. Complete necessary paperwork correctly (incident reports petitions case notes).

Education: Bachelors degree in Human Services or related field required

License/certifications: None required

Experience: Experience working in social services required.

  • Promote a relationship between clients families and additional supports.
  • Be on time to scheduled shifts and trainings.
  • Provide transportation to clients in personal or agency vehicles when necessary for work shopping appointments etc.
  • Maintain a positive and professional working relationship with coworkers agencies and other stakeholders.
  • Maintain a team approach to services.
  • Maintain client rights.
  • Participate in agency committees as assigned.
  • Assure the coordination of services in the community to meet client needs (ie. Financial legal advocacy medical ect.
  • Promote community / family involvement in facility programs.
  • Participate in and promote organizational fundraising and annual events.
  • Participate in the planning and implementation of client activities.
  • Clock in and out per agency policy.
  • Request scheduled and unscheduled PTO utilizing the PayCom program.
  • Follow program and agency procedures for notifying management when unable to attend a scheduled workday.
  • Perform all other related duties as assigned and in accordance with AID policies and philosophies.
  • Promotes client wellness through coaching counseling consultation motivational enhancement and information and referral services to clients facing various health challenges as needed.
  • Communicates with service delivery partners which may include Physicians Nurse Practitioners Counselors Pharmacists Employers Community Supports and other Health Professionals as needed to facilitate the achievement of clients wellness objectives.
  • Utilizes a variety of appropriate resources and educational materials for wellness coaching. (ie. Health Matters Curriculum Smoking cessation materials Weight Management information etc.
  • Promote awareness and respect for the diversity of our clients employees families and other stakeholders.
