Job Purpose
We believe that the childs support system is the most valuable blessing. Therefore we seek to provide physical emotional and spiritual support to the families and the children we serve. Essential to such support is the appropriate and positive interaction between the childs caregivers and the staff. This specific Social Worker supports the Outpatient Clinics of Bethany Childrens Health Center.
Essential Duties & Responsibilities

Percent of Time
The Social Worker is a primary resource and contact for the patient and family throughout the patients stay by providing emotional support crisis intervention advocacy and bereavement services. Documents all interactions with patients and families in the electronic medical record.

Facilitates the preadmission and admission process with the patients family.

Acts as a liaison between interdisciplinary team and patient/ family by facilitating communication answering questions and offering professional guidance during the inpatient stay.

Ensures a safe discharge plan by collaboration with the patient/ family and various disciplines to address all aspects of the patients needs upon discharge.

Follows the patient and family after discharge ensuring satisfaction and continued support.

Qualifications
Education:
Bachelors Degree required. Masters Degree preferred
Experience:
12 years of medical social work or case management experience preferred.
License/Certification/Credential:
LMSW or LCSW preferred

Bethany Childrens Health Center (formerly The Childrens Center Rehabilitation Hospital) located in Bethany Oklahoma is an innovative leader in the field of pediatric rehabilitation and 24hour complex care. The ACHC accredited hospital offers inpatient and outpatient services for children (ages 021 and is the only inpatient pediatric rehabilitation facility in Oklahoma.
