The Diabetes Educator in collaboration with the multidisciplinary teams provides diabetes selfmanagement training and education to patients with diabetes and their families. This includes coordinating care assessing risk factors providing individual and group education sessions developing care plans monitoring and communicating outcomes. CDCES required within 2 years of hire.
Assessment of patients abilities limitations and barriers to diabetes selfmanagement
Collaboration with referring provider and patient care team to identify and prioritize strategies to assist patients with diabetes selfmanagement education
Assist patients with medication adjustments to achieve optimal glycemic control in direct collaboration with the primary care provider
Understand the pathophysiology of diabetes medication management acute and chronic diabetes related complications and motivational interviewing
Use selfmanagement training techniques to assess plan and implement patient and family education in individual and/or group settings. Education to include selfmonitoring of blood glucose medications diet/meal planning exercise goal setting coping skills and reduction of risk factors
Keep current in new and advancing diabetes related technologies
Participate in CQI projects and evaluation of outcome measures to assess diabetes education effectiveness
Applicable Experience:
