Summary

The Nurse Practitioner (NP) provides medical management to patients on the inpatient rehabilitation unit at South University Hospital.

Job Description

Obtains a thorough medical history from patient and dependent on patient population and setting will perform an examination and/or assessment to determine patients needs. With other healthcare professionals analyze and interpret information collected from patient medical records symptoms physical findings or diagnostic information to develop and establish appropriate diagnosis. Acknowledges and demonstrates the importance of care coordination and navigation by rounding on patients. Works in collaboration with providers and independently to provide care. Formulates a plan for treatment of the patient and prescribes medications based on efficacy safety and cost as legally authorized if necessary in order to aid recovery and manage pain. Maintain complete and detailed records of patients health care plans and prognoses in a timely manner. Makes necessary healthcare referrals. Provide patients with information needed to promote health reduce risk factors or prevent disease or disability. Counsels patients and family members about selfmanagement on prevention and treatment plan for health issues tailoring instructions to patients individual circumstances. May participate in clinical research studies. Incorporates evidence based practice guidelines into care. Maintain current knowledge of state legal regulations for advanced practice provider practices including reimbursement of services. Keep abreast of regulatory processes and payer systems such as Medicare Medicaid managed care and private sources as applicable. Demonstrate extensive advanced knowledge of medical principles practices and techniques.

Qualifications

Completion of a masters postmasters or doctorate from an nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission.

Demonstrated current competence and provision of care treatment or services for an adequate volume of patients in the past twelve months or completion of masters/postmasters degree program in the past twelve months. Experience must correlate to the privileges requested.

Current licensure by the applicable state board of nursing for advanced practice. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances.

Certified Adult Gerontology Nurse Practitioner (AGPCNPBC) or (AGNP) or (AGACNPBC) or (ACNPCAG) or Certified Family Nurse Practitioner (NPC) or (FNPBC).

Obtains and subsequently maintains required department specific competencies and certifications.

  • Schedule MFinpatient setting Hospital (no clinic)
  • South University Hospital
  • Experience preferred. New gradsmaybe considered CVs should reflect ability to treat patients in complex setting with many medical issues that may challenge new grads
  • MD supporting at all times.

Duties:

  • Assist in the management of hospital care for rehab patients.
  • Responsible for diagnosis and treatment of complex medical patients in conjunction with the interdisciplinary care team.
  • Works in an interdisciplinary team to provide the highest quality of patient care.
  • Complete medical progress notes daily for the patients assigned to their care.

  • Make Up of Team:
  • The Hospitalist team consists of approx. 60 providers.
  • 55 physicians and 5 APPs.

  • The Rehab team will consist of a Hospitalist physicians and this APP the Rehab physicians and APPs and nursing staff.

  • Treatment and rehabilitation of post trauma stroke severe illness etc. Rehab docs manage the physical medicine rehab while the hospitalists help manage the medical conditions that the patient may also have that may not be directly related to the event requiring rehab.

  • The hospitalist group is an expanding group with a great support system for all of the members. An applicant here will find that the group is amazing at supporting their colleagues and there are plenty of people to direct questions to. The group is consistently one of the top performing and scoring provider groups in Sanford for provider engagement freedom of opinion and peer relationships.
