Hours: 21 hours per week
Salary: 29633.89 FTE which equates to 16594.98 for 21 hrs pw
Permanent/Parttime
CHS Community Support Services work with vulnerable people who are homeless or at risk of homelessness to promote independent living skills. The services also work with families/relatives to prevent homelessness and they support people to sustain their tenancies where they are in independent accommodation.
What Youll do
In this varied role you will provide the highest quality support to young people aged 16 25 to enable them to achieve their personal aspirations and live as independently as possible
You will do this by working closely with other agencies; keyworking an allocation of service users meeting them regularly to develop progress and review their individual action plan to support them achieving their goals and independent living skills. This includes coping strategies maintaining their physical and mental wellbeing tenancy sustainment obtaining/maintaining employment education or training and social activities.
You will also prepare accurate written and electronic records of daily events support sessions action plans Rickter Assessments and support reviews etc.
