BAYADA Home Health Care is currently seeking an experienced Medical Social Worker MSW LMSW for a full time opportunity performing hospice visitsthroughout Bladen Cumberland Harnett Hoke Moore Robeson and Sampson Counties.

  • Masters Degree in Social Work required
  • NC MSW license required
  • Ability to travel throughout entire territory required (we offer mileage reimbursement)
  • Prior medical experience required

Prior hospice experience preferred but not required. As a Medical Social Worker you will be an integral member of a multidisciplinary health care team that provides skilled nursing and rehabilitative care to clients affording them the opportunity to receive the medical care required to remain at home.

Responsibilities include:

* Assessing emotional factors related to terminal illness.
* Assisting the physician and other IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
* Assessing the patient/family psychosocial status.
* Assessing environmental resources and obstacles to maintaining safety.
* Participating in the development and revision of the plan of care.
* Preparing clinical and progress notes.
* Identifying family dynamics and communication patterns.
* Identifying and utilizing appropriate community resources and assessing client/family ability to
access them.
* Assessing caregivers ability to function adequately.
* Identifying support systems available to reduce stress and facilitate coping with endoflife care.
* Evaluating for longterm care when appropriate and assessing ability to accept change in level of care.
* Evaluating for respite care when appropriate and coordinating respite stays.
* Assisting client/family in assessing financial resources when appropriate.

Qualifications for the ideal candidate:

  • Have a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education
  • Have a Masters of Social Work license (LMSW)
  • Meet all state practice requirements
  • Have a minimum of one year experience in social work preferably hospice or palliative care
  • Have a criminal background check completed
