Are you a dedicated Physical Therapist looking to make a meaningful impact in patients lives Were seeking an experienced PT to join our premier home health team in Sacramento delivering personalized rehabilitation services that help patients thrive in their own homes.
Position Overview
As a Home Health Physical Therapist youll provide expert therapeutic care while enjoying the autonomy and flexibility of managing your own patient caseload. This role offers competitive compensation $105000$150000 annually) and the opportunity to build lasting relationships with patients and their families.
Essential Responsibilities
- Perform comprehensive physical therapy evaluations and functional assessments
- Develop and implement evidencebased treatment plans aligned with patient goals
- Deliver handson therapeutic interventions and exercise programs
- Provide expert guidance on mobility safety and injury prevention
- Document patient progress and maintain detailed electronic health records
- Coordinate care plans with physicians nurses and other healthcare providers
- Demonstrate excellent time management while traveling between patient homes
Required Qualifications
- Masters degree in Physical Therapy from an accredited program
- Current Physical Therapy license in California
- Minimum 1 year of home health PT experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong interpersonal and communication skills
- Proficiency in electronic medical record systems
