Job Description
Registered Nurse (RN) Patient Care Coordinator
Join our dynamic healthcare team as a Registered Nurse where youll play a vital role in delivering exceptional patient care in a collaborative supportive environment. Were seeking a compassionate and skilled RN to provide comprehensive nursing care while coordinating with interdisciplinary teams to ensure optimal patient outcomes.
Core Responsibilities:
- Conduct comprehensive agespecific patient assessments and develop individualized care plans
- Lead interdisciplinary care coordination and implementation of evidencebased treatment plans
- Deliver and document patient care using EPIC electronic medical records system
- Administer medications and treatments with precision and accuracy
- Monitor and evaluate patient progress adjusting care plans as needed
- Collaborate with healthcare team members to ensure optimal patient outcomes
- Provide patient and family education regarding treatment plans and health maintenance
- Maintain compliance with all healthcare regulations and facility protocols
Required Qualifications:
- Graduate of an accredited professional nursing program
- Current Virginia RN license or eligibility to obtain licensure
- Current BLS/CPR certification
- Proficiency with electronic medical records (EPIC)
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
Preferred Qualifications:
- ACLS PALS and/or NRP certifications
- Previous acute care nursing experience
- Bachelors degree in Nursing (BSN)
